Here is a simple drawing i made. It is not to scale or size i just drew it to show you how you could make it out of 3/4 ply.

Here is how you do it. See the straight lines? take a ballnose bit and route that down say a 1/8th or 1/4th deep on both pieces.

Cut the center out

Cut out your hooks.

Cut it out from the material

Take a dowel and put it thru the hole in the hook

lay the dowels in the routed straight line with the hook on it.

Take the second piece and lay it on top so the routed lines line up to create a round hole for the dowel.

Screw the two pieces together and you're done!
