Has anyone put together a shopping list of parts necessary to build a rotary axis, including some instructions?
I know there are web sites for building CNC routers. I thought there might be resources for rotary axis.

I did that a while back and it worked out pretty nicely. Here is a link to the thread

I bought a harmonic drive knock-off several years ago and it works fine. No shopping list but similar to this one (https://www.ebay.com/itm/CNC-K11-100mm-4th-100-1-50-1-Gapless-harmonic-Reducer-Rotation-3-Jaw-Tailstock/253934566738?hash=item3b1fadfd52:g:cbIAAOSwV45bxDN W:rk:1:pf:0) with 1:50 reduction. For larger diameters >3" I think this is preferable to the belt drives. For small diameters like pen work the lower gear reduction of the belt drive is surely enough.
Only modification was installing a Oneway 4-jaw "Talon" wood lathe chuck and mounting the gearbox on plates that can be installed/removed from my t-slot table quickly. I know some of the eBay sellers offer the complete solution already with a larger 4-jaw chuck (adapting this after the fact was difficult).
