This has been troubling me as of late. I don't get how this is possible, but it is what is happening.

I have a 5x10 prt with 3 hp columbo and g4 upgrade. I have tried squaring the y to the x rails, also squaring to my mdf table base material, which is square. I run a bit along the length of material on x, and there might be a 1/16" difference. Y is fine. At this point, the little cutout (pictured is out about perhaps 3/16"

any ideas?

Johnny,
One X motor is ahead of the other X motor. In this case it appears that the X motor at the bottom of your illustration is ahead of the other one. The squareness of the base sheet is not important. What is important is that both motors match in physical X location.

The easiest way I have found to 'tweak' the gantry back to square is to clamp one side of the gantry to lock it into place and then 'bump' the gantry in one direction to let the one motor pull the gantry where it needs to be.
