Anyone using madcam on a regular basis? My limited tinkering is showing that all measurements are in metric, am I correct? If so, is there a way to convert to inch - without manually converting?

If your Rhino drawing is in inch units your madcam toolpath will be in inches. So if you want .5 inch cutter just enter .5 for the tool.
(tip- In Rhino set mesh tolerance to something reasonable for the size item you are cutting, such as .01 for a furniture part)
